How do I know my plan covers Mental Health services?

All the medical plans we work with have mental health coverage. However, to better assist you, you can contact the number on the back of your medical plan card.

What documents should I bring on the day of the appointment?

If you are an adult, bring the following: photo ID (election card / driver’s license). The MA-10 if you do not have the health plan card. If you are a minor, you must bring MA-10 and ID of mom or dad, birth certificate and vaccination card or papers. Since it is a minor, both mom and dad must go, if one cannot show up, you must bring evidence of custody. You can call again for more details to the number on the back of your medical plan card.

What should I do if I cannot go to my appointment?

If it is a follow-up appointment, you must call 787-641-9133 to request a new appointment. If it is for an initial appointment you should contact the telephone number on the back of your medical plan card, and select mental health at the prompt.

Do my services cover hospitalization for mental health?

Yes, the PSG covers hospitalization services, as well as the private medical plans and advantage that we work.

What’s an MA-10?

It is the document provided by the Medical Assistance offices to confirm the eligibility of the medical plan prior to the delivery of the card.

Besides the MA-10, what other documents do I have to bring to my initial appointment?
